How do I flag a procedure or remove my flag?

There may be occasions when an institution inadvertently publishes a duplicate procedure or a procedure with incomplete or incorrect information.


When any CUSP user encounters such a procedure, they can flag the procedure to be reviewed by a site administrator (specifically, a member of the CUSP steering committee quality control group) who can work with the publishing institution to make any needed corrections.


With regard to procedure content, only procedures with missing or misstated content that affects understanding the procedure should be flagged. For example:

  • The procedure has Bob's grocery list instead of an actual procedure description.
  • The procedure is missing a procedure description.
  • The dosing is incorrect (e.g., says 5mg instead of 5mcg)


Procedures with grammatical or spelling errors that don't affect understanding the procedure should not be flagged. For example:


  • A word is misspelled or repeated.
  • There is a simple typo, but it doesn't affect readability (i.e., within context, it's still understandable).
  • There is a grammatical error (e.g., tenses, subject/verb agreement, etc.).


To add a flag to a procedure: 

  1. Open any procedure from the Procedures list to view it (see View/Read a Procedure, if needed). 
  2. Within the Procedure Viewer (shown below), click on the Flag button (flag icon) to flag the procedure.

  3. A "Reason for Flag" pop-up will appear, as shown below:

    1. To proceed with the flag:
      1. Type in the reason why you are flagging the procedure (e.g., this procedure is exactly the same as Procedure #123). Note: This information will be viewable by all site users (as it's meant to be helpful with regard to use of the procedure).
      2. Click SUBMIT.
      3. When the flag is added:
        1. An email is sent to the CUSP Quality Control team (cuspqc@thefdp.org)
        2. The flag button will fill in with shading, like this:

        3. The Procedure Viewer will display a message with the added flag, like this (note: :

    2. If you decide not to proceed with the flag:
      1. Click CLOSE (the Flag button will not fill in with shading and no procedure flag message will appear)


To edit or remove the flag on the procedure (e.g., to add additional information or if the flag was added on the wrong procedure):


  1. Reopen the flagged procedure from the Procedures list to view it (see View/Read a Procedure, if needed).
  2. Click on the shaded Flag button (flag icon)

  3. The "Reason for Flag" pop-up will reappear and will display your previously entered text.

    1. To edit the reason for flag text:
      1. Make the desired changes.
      2. Click SAVE.
    2. To close the flag without making any changes (the flag will remain on the procedure as is):
      1. Click CLOSE.
    3. To remove the flag:
      1. Click WITHDRAW.
      2. Click REMOVE FLAG on the Remove Flag pop-up. (Clicking CANCEL will return you to the "Reason for Flag" pop-up.)
